Orthopaedic surgery: Cruciates, the lowdown

Cranial cruciate ligament disease is one of the most common orthopaedic conditions of the dog. Despite this, we do not have a clear understanding of the pathology of canine cranial cruciate ligament disease. In addition, the perfect treatment option still eludes us. The ideal treatment would restore the health and function of the cranial cruciate ligament and prevent progressive stifle osteoarthritis. But how far away are we from this? This lecture discusses the biomechanics of the cranial cruciate ligament, which does much more than simply prevent cranial tibial translation. We examine and critique the recent evidence to support methods of static and dynamic stifle stabilisation, including the lateral fabello-tibial suture, TightRope, tibial plateau levelling osteotomy (TPLO), and tibial tuberosity advancement (TTA). The menisci play an important role in stabilisation and load distribution within the stifle joint. The consequences of partial meniscectomy, total meniscectomy, and meniscal release are discussed. Finally, recent experimental studies evaluating synthetic intra-articular ligament reconstruction and bio-integrated ligament implants are discussed. The aim of this presentation is to provide a relevant overview of cranial cruciate ligament disease, meniscal injury, and the latest research on comparative outcomes of stifle stabilisation.
