Pituitary pars intermedia dysfunction - revitalising the geriatric horse

This presentation will describe the classic and subtle clinical signs associated with pituitary pars intermedia dysfunction (PPID) in horses. This syndrome was previously known as equine Cushing’s syndrome, but many differences exist with pars intermedia origin PPID in horses and Pars distalis or adrenal origin Cushing’s disease in humans and dogs. Diagnostic tests such as endogenous adrenocorticotropic hormone (ACTH), ACTH response to thyroid releasing hormone (TRH) stimulation and dexamethasone suppression test will be compared and contrasted for various clinical situations. Practical considerations such as sample handling and seasonal variations will be discussed. Response to treatment with pergolide and problems associated with use of compounded formulations will be described. Aims: Participants should have a comprehensive understanding of: 1. Subtle and advanced clinical signs of PPID 2. Advantages and disadvantages of various tests used to diagnose PPID 3. Why serial cortisol or T3/T4 concentrations are not useful for diagnosis of PPID 4. Screening tests for older clinically normal appearing horses 5. Treatment options for PPID 6. Problems associated with use of compounded pergolide formulations
